Home Depot offers an adoption reimbursement program to eligible associates. To request more information and a reimbursement application, log onto
livetheorangelife.com and click on the Health Insurance & Wellness Tab. Then click on Take Action > Request Materials. Check the Adoption Reimbursement Application box and click the Request Materials button at the bottom of the page. You may also call the Benefits Choice Center at 1-800-555-4964 to request these materials.

Update your W-4 tax elections for
Federal,
State and
Local withholding if appropriate.
